Waiver and Release of Liability

As the parent or legal guardian of the above-named participant, I recognize and acknowledge that there are certain risks of physical injury associated with participating in sports activities, including the basketball program offered by LJ Elite. These activities may involve, running, jumping, physical contact with other participants, and the risk of injuries from playing equipment.

In consideration of the Academy allowing my child to participate in its basketball program, I hereby agree to assume the full risk of any injuries, damages, or loss that my child may sustain as a result of participating in any and all activities connected with or associated with such program.

I hereby waive and release any and all claims I may have, or that may arise against LJ Elite, its affiliates, subsidiaries, directors, officers, employees, volunteers, and agents, from any and all claims for injuries, damages, or loss that my child may have or which may accrue to my child arising out of, connected with, or in any way associated with the basketball program.

Medical Emergency Treatment Authorization

In the event of an emergency, I authorize LJ Elite to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for my child's immediate care and agree that I will be responsible for payment of any and all medical services rendered.

Photo/Video Release

I hereby grant LJ Elite the right and permission to use photographs and/or video recordings of my child on its website/Social media and in publications, promotional flyers, or for any other similar purpose without compensation to me or my child.

Acknowledgment of Understanding

I have read this Waiver and Release of Liability form, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
